COMMUNITY CONNECTIONS PARTNERSHIP
Community Connections Partnership (CCP) is a non-profit organization in the Minneapolis/St. Paul metro area providing support to people with disabilities. We specialize in the areas of employment, skill development, healthy lifestyles, community supports, and recreation opportunities. Our dedicated staff are committed to uncovering each person’s potential and supporting them step by step in their growth and development. We work in a partnership with the people we support and their team members to achieve the most meaningful life possible.
WHO USES OUR SUPPORTS?
All of Community Connection Partnership supports are open to adults with intellectual (developmental) disabilities and/or mental illness. Many of the people we serve also need support with challenges such as autism spectrum disorder, severe and persistent mental illness, as well as chronic illness or physical impairments. No matter what a person’s diagnosis needs are, we believe everyone has something to contribute and seek to create a supportive environment where this can happen.
Funding Sources: Our supports are available to people living in the Twin Cities metro area who receive funding through Private Pay, Medical Assistance, County, Home and Community Based Waivers, including Brain Injury (BI), Community Alternatives for Disabled Individuals, Developmental Disabilities (DD).
