Community Support Consultant

Community Connections Partnership's Community Support Consultants/Direct Support Professionals are at the heart of the exceptional service we offer to individuals with disabilities. Every day our Community Support Consultants dedicate their creativity and energy to helping the people we serve develop a higher level of independence and job related skills. Whether it’s working with a community employer, developing sensory skills, or assisting the manager, the Community Support Consultant II is there to guide and assist. Positions are available in the Hennepin County areas, Monday-Friday day hours.

The Community Support Consultant position is ideal for people who are:

PAID COMMUNITY SUPPORT CONSULTANT TRAINING INCLUDES:

  • Medication Administration
  • CPR & First Aid
  • Person Centered Training
  • Supported Employment Training
  • Positive Support Rule
  • Specialist On-Site Orientation
  • Shadow Shifts

DAILY JOB DUTIES MAY INCLUDE:

  • Encouraging and respecting personal choice and decision making
  • Documenting daily progress
  • Developing and assisting individuals with educationally based assignments such as group discussion and community based activities
  • Administering medication
  • Assisting individuals in developing and mentoring appropriate and professional workplace interactions
  • Assisting individuals in the development and support of work skills, both on-site and in the community
  • Transporting individuals to and from work on a daily basis
  • Ensuring that the environment is safe and clean
  • Assisting with personal cares as needed
  • Implementing therapeutic intervention
  • Other duties as assigned

POSITION REQUIREMENTS:

  • Possess a valid driver’s license
  • Pass a motor vehicle record check with an acceptable driving record
  • Possess a reliable vehicle
  • Ability to transport up to three people in your vehicle at one time
  • Pass the Department of Human Services background check
  • Ability to communicate effectively in the English language, oral and written
  • Experience with individuals with disabilities preferred
  • High school diploma or equivalent
